MID #, Serial #, or Location

The MID# can be found on the red Modus sticker on the front of the machine. The serial # can be found on the manufacturer's barcode sticker. If you cannot find the MID # or the Serial #, please include a referenceable location within the office (e.g., 1st Floor Reception Desk).

Request

For SUPPLIES, please include the name of the supplies as well as the quantity. For SERVICE, please include a description of the issue as well as any pertinent information for a technician to access the machine for repairs (office hours, access codes, etc.). If you have an image to further describe the issue, please send it via email to SUPPORT@modussolutionsllc.com.
